Thank you for your interest in Roberto Cruz Leadership Academy. On behalf of all of us, we hope that you and your loved ones are well. We know that you have a lot to focus on and are doing your best to plan for the future. An important part of that future will include the high school education of your child. That is why we want to make the registration process easy for your family.
TO RESERVE YOUR CHILD’S ENROLLMENT FOR FALL 2020-2021, PLEASE FILL OUT THE INTENT TO ENROLL FORM.
**If you wish, you can register your child from home using any of the options below. Note: All documents [Birth Certificate, and Immunization Record, & if applicable, Individualized Education Plan (IEP), 504 Plan, and/or any Student Support Information] can be sent to us when the “Shelter in Place” order is lifted. Not being able to attach documents will not disqualify your Registration Forms. Please call (408)471-4680 Ext. 4002 if you have any questions or need assistance.**
OPTION A: ONLINE - Registration Forms Available on Docusign Platform
OPTION B: BY MAIL
Step 1: Either Call or Email the Director at
(408)471-4680 Ext. firstname.lastname@example.org to request a Registration
Forms to be mailed to your home. IMPORTANT: Be sure to include your home address.
Step 2: Complete the Registration Forms in pen.
Step 3: Mail the completed Registration Forms along with your child’s
Birth Certificate, and Immunization Record. ** if applicable,
please include Individualized Education Plan (IEP), 504 Plan, and/or any Student Support Information.
Note: All documents besides the Registration Forms can be
sent to us when the “Shelter in Place” order is lifted.
